Large institutional buyers of TB drugs, including international foreign aid agencies and global trust funds, regularly work together to understand their pooled demand. However, by expanding this circle to include high-burden national programs and large regional or subnational programs, a more ambitious pooling approach could help to bring visible demand to these manufacturers closer to the actual need for these drugs on the part of hundreds of thousands of patients around the world. This would require a serious, verifiable commitment to increase access to care in many settings to have the desired effect, and innovative financial instruments to appropriately distribute risk and investment among partners. A stronger supply chain with multiple 3rd party logistics providers working on a regional basis was also identified as a potential positive outcome of this ambitious demand pooling as well.
